An elderly or disabled person needs to feel safe and remain close to other people. Without sufficient support, patients may have difficulty sleeping, poor appetite, or an inability to concentrate. Personal care by Minnesota Quality Care will be designed to address the vulnerability, loneliness, boredom, and isolation often felt by elderly or disabled people. Engage with Minnesota Quality Care for a service experience that centers around you or your loved ones. We ensure your loved ones feel safe, supported, and empowered in every aspect of your life.

  • Financial Management Services

    For those looking to self-direct their services through consumer directed community supports (CDCS) or the Consumer Support Grant (CSG), Aspen FMS provides help with financial tasks, billing, and employer-related responsibilities. This means Aspen’s role is to support your loved one to fulfill his or her responsibilities in being the employer of his or her workers.

  • Personal Care Services

    Personal Care Assistants (PCAs) are provided by Minnesota Quality Care for seniors in need. PCAs help with the activities of daily living that keep your loved one safe in their home. They assist clients with eating, bathing, walking from room to room, and running errands. PCA workers keep clients safe in their homes by being there to help with daily tasks.

  • CFSS ( Community First Services & Supports)

    Community First Services and Supports is designed to accomplish many of the same goals as the PCA program, and even more. In addition, the eligibility requirements for CFSS will remain the same as they were for PCA. CFSS covers activities of daily living (ADL), instrumental activities of daily living (IADL), health-related tasks, and observation and redirection of behavior.

  • 245D Basic Waivered Services

    To improve the dignity, health, and independence of those we serve, we offer 245D services to people with disabilities and those over the age of 65. These services can provide the level of assistance and supervision that is necessary to ensure the health and safety of your loved one. This includes ICLS, Homemaker Services, Personal Support, or Respite Care Services.

  • Social Services Support

    We do so much more than home care services for our clients. When you need help with paper work and referrals we are there to help as part of your health care team. Our office staff has decades of experience working with county and health plan staff. We provide custom training for each PCA and Homemaker for the unique needs of each client.
