What is CFSS?

Consumer-Focused Support Services designed to help individuals live independently in their own homes

Independence & Everyday Support

CFSS helps people of all ages live comfortably at home and in their communities. You'll get support with everyday activities like eating, bathing, grooming, shopping, cooking, managing medications, and more—so you can focus on living your life with greater independence.

More Options, More Control

With CFSS, you get to choose how your care is delivered. Pick between working with a provider agency or taking charge as the employer of your own support workers—including family, friends, or even your spouse or parent. You decide who helps you and how.

Personalized Plans & Extra Supports

Create a care plan that truly fits your needs. CFSS lets you buy helpful goods and services, including a personal emergency response system, to make daily life easier. You'll also get guidance from a consultation provider to explore your options and set up the best plan for you.

CFSS Transition

Important information about the transition from PCA to CFSS services

CFSS Is Here!

In February 2024, the federal government approved the transition from PCA to CFSS. Minnesota Quality Care and the Department of Human Services are working to ensure a smooth change that provides more freedom, flexibility, and control over home care services.

Replacing PCA

CFSS will replace PCA and the Consumer Support Grant (CSG) in Minnesota. If you currently receive PCA services, you'll switch to CFSS at your next annual reassessment. As long as you remain eligible, CFSS offers added benefits for both care recipients and family caregivers.

Expanded Opportunities

CFSS offers a wider range of options for continuous support, including training for caregivers, acquisition of essentials, and choice between agency care or budget care options.

Frequently Asked Questions

Find answers to common questions about CFSS services

What is CFSS and who is eligible for it?

CFSS (Community First Services and Supports) is a Minnesota health care program that helps people with disabilities, chronic illnesses, or age-related needs live independently at home or in the community. To qualify, you must be enrolled in Medical Assistance and need help with daily living tasks, similar to PCA (Personal Care Assistance) requirements.

How is CFSS different from PCA services?

CFSS offers more flexibility and choice than PCA. With CFSS, you can hire your own support workers—including family, friends, or your spouse/parent—create a personalized service plan, and choose between an agency provider model or managing your own care budget.

What kinds of services does CFSS cover?

CFSS covers assistance with daily living activities (like bathing, dressing, eating), health-related tasks, help with medications, shopping, cooking, laundry, and even redirection or behavioral support. You can also use your budget to purchase certain goods and services to help you stay independent.

Can I customize my CFSS services to fit my unique needs and preferences?

Yes! One of the biggest benefits of CFSS is the ability to personalize your care. You work with a consultation provider to create a service plan tailored to your daily routines, health needs, and goals. You can choose who supports you, what tasks are prioritized, and even purchase goods or services that make life easier for you.

How do I get started with CFSS or switch from PCA?

If you're interested in CFSS or transitioning from PCA, contact us and we'll guide you through the process. We'll help you with eligibility, paperwork, building your service plan, and choosing the right service model for your needs.
