Please use this form to submit your complaint. Complaints from recipients and/or caregivers will be thoroughly investigated and resolved within 30 days. If resolution cannot be achieved within this timeframe, an explanation for the delay will be provided. Public complaints will be investigated at Minnesota Quality Care’s discretion and referred to the appropriate controlling agency if necessary.
