Our 245D Basic Waiver Services provide the level of assistance, supervision, and care that is necessary to ensure the health and safety of Minnesota’s seniors. We’re committed to delivering personalized support that respects each individual’s dignity and promotes their independence.
The HCBS standards under Minnesota Statutes, chapter 245D, are part of a larger HCBS Waiver Provider Standards initiative to create meaningful improvements in the lives of those we serve through PCA and CFSS services.

We currently provide the following 245D Basic Waiver Services to our clients
For people who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes.
Example services include:
Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living.
Service examples:
Provides overnight assistance and monitoring by an awake staff in the person's home.
This service includes:
Services provided in the person's home or community in support of achieving potential, increasing independence and meeting community inclusion goals.
Services may include:
Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.
Our respite services offer:
Individualized home supports cover community living services in four categories: Community participation, Health/safety/wellness, Household management, and Adaptive skills.
