Enhancing Care Through 245D Waiver Services

Our 245D Basic Waiver Services provide the level of assistance, supervision, and care that is necessary to ensure the health and safety of Minnesota's seniors. We're committed to delivering personalized support that respects each individual's dignity and promotes their independence.

The HCBS standards under Minnesota Statutes, chapter 245D, are part of a larger HCBS Waiver Provider Standards initiative to create meaningful improvements in the lives of those we serve through PCA and CFSS services.

Eligibility Requirements

Minnesota seniors and individuals with disabilities may qualify for our 245D services

Our Covered Services

We currently provide the following 245D Basic Waiver Services to our clients

Individual Community Living Supports (ILCS)

For people who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes.

Example services include:

  • Daily living skill reminders
  • Medication monitoring
  • Safety supervision
  • Transportation assistance
  • Meal preparation guidance

Homemaker Services

Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living.

Service examples:

  • Light housekeeping and cleaning
  • Laundry and linen care
  • Meal planning and preparation
  • Grocery shopping assistance
  • Home organization support

Night Supervision

Provides overnight assistance and monitoring by an awake staff in the person's home.

This service includes:

  • Overnight safety monitoring
  • Assistance with nighttime routines
  • Response to nighttime needs
  • Medication reminders
  • Emergency preparedness

Personal Support

Services provided in the person's home or community in support of achieving potential, increasing independence and meeting community inclusion goals.

Services may include:

  • Community outings and socialization
  • Skill-building activities
  • Personal care assistance
  • Transportation support
  • Recreational activity support

Respite Care Services

Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.

Our respite services offer:

  • Temporary relief for primary caregivers
  • Continuity of care during caregiver absence
  • Emergency respite options
  • Specialized care for unique needs
  • Flexible scheduling options

Individualized Home Supports

Individualized home supports cover community living services in four categories: Community participation, Health/safety/wellness, Household management, and Adaptive skills.

Service components:

  • Customized support plans
  • Skill development training
  • Health and wellness coaching
  • Home management assistance
  • Behavioral support strategies
